  • Breast Cancer & HRT

    What is New?
    Hormone replacement therapy (HRT) was once routinely used to treat menopausal symptoms and to prevent heart disease. But its use changed abruptly when a large clinical trial found that the treatment actually posed more health risks than benefits, particularly when given to older postmenopausal women. When taken for more than 5 years, HRT increases the risk of breast cancer, a finding confirmed in multiple studies of different HRT combinations. HRT – particularly estrogen combined with a progestin, can make your breasts look more dense on mammograms, making breast cancer more difficult to detect. However, further review of clinical trials and new evidence shows that hormone therapy may be a good choice for certain women, depending on their individual circumstances.
    Who can benefit from hormone therapy?
    Women who experience early menopause, particularly those who had their ovaries removed and don’t take estrogen therapy until at least 45, have a higher risk of Osteoporosis, coronary heart disease, Parkinson’s-like symptoms and anxiety or depression. Despite the possible health risks, HRT is still the most effective treatment for menopausal symptoms. The benefits may outweigh the risks if you’re healthy and:
    • Experience moderate to severe hot flushes or other menopausal symptoms
    • Have lost bone mass and can’t tolerate or aren’t benefitting from other treatments
    • Stopped having periods before age 40 (premature menopause) or experienced premature ovarian insufficiency before age 40
    Who should avoid hormone therapy?
    Women with a current or past history of breast cancer, ovarian cancer, endometrial cancer, blood clots to the legs or lungs, or stroke should usually not take HRT. In general, it is not recommended for women over 60. Smoking is strongly discouraged. Women who aren’t bothered by menopausal symptoms and started menopause after age 45 do not need hormone therapy to stay healthy. Instead, talk to your doctor about strategies which might include lifestyle changes and medication. Keeping cool, limiting caffeinated beverages and alcohol, and practising deep breathing or relaxation techniques can also be very helpful. Alternative approaches such as tai chi, yoga and acupuncture can be tried. For vaginal dryness or painful intercourse, a lubricant may provide relief.
    The bottom line: Hormone therapy isn’t all good or all bad
    To determine if HRT is a good treatment option for you, talk to your doctor about your symptoms and health risks. Be sure to keep the conversation going throughout your menopausal years. As researchers learn more, recommendations may change.

    Treatment options when cancer spreads to the brain
    In some people diagnosed with cancer, there is the unfortunate potential for the cancer to spread to the brain.  Although any type of cancer has the ability to spread to the brain, brain metastases tend to arise from lung cancer, breast cancer and melanoma.
    Treatment for brain metastases includes surgery, radiation therapy to the whole brain, and a relatively new option called stereotactic radiosurgery.  The use of chemotherapy is controversial and not routinely used for most brain metastases due to the difficulty in getting these drugs to enter the brain.  Trials of new drugs that cross into the brain, or use the body’s own immune system to fight the cancer, are ongoing.  Surgery and radiation therapy therefore remain the current standard treatment options.
    The choice of treatment depends on multiple factors including the extent of the disease, the person’s age and how well they are functioning.
    What is stereotactic radiosurgery?
    Stereotactic radiosurgery (SRS) is an alternative to invasive surgery.  It is most suited to people who are unable to undergo surgery, or for tumours and abnormalities that are hard to reach or located close to vital organs.  It can also be used to treat some non-cancerous lesions such as arteriovenous malformations and vestibular schwannomas.
    SRS treatment uses image-guided radiation therapy.  Images of the location of the tumour immediately before, and sometimes during  radiation delivery, improve the precision of the radiation beams. The beams are able to target the tumour from multiple angles and spare nearby normal tissue.
    Benefits of stereotactic radiosurgery
    Treatment with SRS is shorter than for other methods.  Treatment is delivered as a single high-dose, or two to five medium-doses of radiation.  Side-effects are generally less than with other options. SRS has also been shown to enhance survival in people with a single brain metastasis.
    The key is to ensure that people with newly diagnosed brain metastases are referred to a radiation oncologist for an opinion.

  • Shady plan to give Maylands a boost

    THE Guildford Road side of the Maylands town centre is about as green as Death Valley, prompting local Elli Petersen-Pik to start a tree-planting campaign.

    “It’s not a pretty streetscape, it’s not very inviting,” Mr Petersen-Pik says.

    “It’s not responsible or acceptable that not even one street tree has been planted to shade pedestrians along that street.”

    He says the lack of trees will hamper any attempts to revitalise the area, while the blaring sun can deter potential customers from walking into town, especially seniors.

    Inviting

    “In East Perth for example, on Royal Street, there’s lots of shade there.

    I’m not necessarily saying we should have plane trees, because they’re not native, but that’s an example of a street that looks beautiful and inviting.”

    The president of the Maylands Ratepayers and Residents Association, at Bayswater council’s AGM last year, Mr Petersen-Pik submitted a motion calling for street trees along that stretch, and says it was encouraging councillor Chris Cornish was his seconder. That’ll go to council this week for a decision.

    “I want to get that issue on the agenda with the city and Main Roads,” Mr Petersen-Pik says.

    “But to get that initiative off the ground is not going to be easy because both of them have to work together which is the complicating situation here.

    Pressure

    “I do want to see the council put more presure on Main Roads to prioritise this issue.”

    The state government plans to widen the Guildford Road reserve in the future, but Mr Petersen-Pik says he’s got plenty of spots for trees that wouldn’t be affected.

  • Labor commits to Carter wetland buyback

    LABOR has pledged $1.5 million towards buying the “Carter’s block” wetlands.

    The privately owned wetlands was partially cleared last year to make way for a housing development.

    Bayswater council’s already pledged $1.5 million and wants the WA Planning Commission to chip in the rest.

    On Wednesday January 25 Labor’s Lisa Baker met with shadow planning minister Rita Saffioti at the site to announce the extra funding.

    Catastrophic

    Ms Baker linked the planning commission’s approval of the clearing to wider “catastrophic failures of the Liberal-National government’s planning and environment policies,” allowed by limp environmental protection laws.

    Bayswater council has petitioned the WAPC to rezone the land as a parks and recreation reserve, and if it can buy the land plans to clean it up and rehabilitate the partly-cleared area.

  • Ladder reprieve

    FITNESS junkies have scored a win over weary neighbours following a decision by the lands minister Terry Redman to keep Jacobs Ladder open without restrictions.

    The hideously steep staircase is a popular exercise spot for early-birds, but was last year threatened with a  curfew after neighbours complained about the noise.

    The City of Perth acknowledged the minister’s decision on its website and included a promise to minimise any impacts on residents.
